1. SPONSOR
The RexMD® GameDay Sweepstakes (“Sweepstakes”) is sponsored by LifeMD, Inc. d/b/a RexMD (“Sponsor”), 236 5th Ave, 4th Floor, New York, NY 10001.
2. ELIGIBILITY
Open only to legal residents of the fifty (50) United States and the District of Columbia who are eighteen (18) years of age or older at the time of entry. Employees, officers, directors, and agents of Sponsor, its affiliates, subsidiaries, advertising and promotion agencies, and immediate family members (spouse, parents, siblings, and children) and household members of each are not eligible. Void where prohibited.
3. SWEEPSTAKES PERIOD
The Sweepstakes begins at 12:00:00 AM Pacific Time (“PT”) on October 15, 2025 and ends at 11:59:59 PM PT on January 15, 2026 (“Promotion Period”).
4. HOW TO ENTER
During the Promotion Period, eligible participants may enter in one of the following ways:
(a) Online Entry: Visit https://www.rexmd.com/biggame and complete the entry form with your full name, email address, state of residence, and age confirmation.
(b) Alternate Method of Entry (AMOE): To enter without using the website, legibly hand-print your full name, complete address, phone number, email address, and date of birth on a 3"x5" card and mail it in a standard envelope to:

Mail-in entries must be postmarked by the end date and received within five (5) days thereafter. Each envelope counts as one entry. Limit one (1) entry per person, regardless of entry method. Duplicate, automated, or bulk entries will be void.
5. PRIZE AND APPROXIMATE RETAIL VALUE (“ARV”)
One (1) Grand Prize:
A trip for two (2) to the 2026 professional football championship game scheduled to be held in Santa Clara, California.
Prize includes:
Two (2) tickets to the 2026 professional football championship game (for winner and one guest)
Round-trip economy airfare for two (2) from a major U.S. airport to the San Francisco Bay Area, CA
Two (2) nights’ standard hotel accommodations (one room, double occupancy)
Approximate Retail Value (“ARV”): $4,995.00 USD
Actual value may vary based on winner’s location and market conditions at the time of booking. Any difference between the stated ARV and actual value will not be awarded. Prize is non-transferable and may not be substituted or redeemed for cash, except at Sponsor’s discretion. If the event is canceled, postponed, or moved, Sponsor reserves the right to award a substitute prize of equal or greater value.
6. WINNER SELECTION AND NOTIFICATION
On or about January 15th 2026, one (1) potential winner will be selected in a random drawing from all eligible entries received.
The odds of winning depend on the total number of eligible entries.
The potential winner will be notified via email and/or phone and must respond within five (5) business days to claim the prize.
Failure to respond within that period or failure to comply with these Official Rules may result in forfeiture of the prize and selection of an alternate winner.
The winner will be required to complete, sign, and return an Affidavit of Eligibility, Liability Release, and Publicity Release (where lawful) within seven (7) days of notification.
7. GENERAL CONDITIONS
All federal, state, and local laws apply. Sponsor reserves the right, in its sole discretion, to cancel, suspend, or modify the Sweepstakes if fraud, technical failures, or other factors beyond Sponsor’s control impair the integrity of the Sweepstakes. Sponsor is not responsible for any incorrect or inaccurate information, lost or late entries, or any errors or malfunctions of any kind.
8. TAXES
All federal, state, and local taxes associated with the acceptance or use of the prize are the sole responsibility of the winner. Sponsor will issue an IRS Form 1099 for the total value of the prize if required by law.
9. PUBLICITY RELEASE
Except where prohibited, acceptance of the prize constitutes consent for Sponsor and its designees to use winner’s name, likeness, and city/state of residence for advertising, promotional, and publicity purposes in any media, worldwide, without additional compensation.
10. LIMITATION OF LIABILITY
By participating, entrants agree to release and hold harmless Sponsor, its subsidiaries, affiliates, advertising and promotion agencies, and each of their respective officers, directors, employees, and agents from any and all liability, loss, or damage arising from or in connection with participation in the Sweepstakes or the acceptance, use, or misuse of any prize.
Sponsor makes no representation or warranty concerning the prize. All trademarks are the property of their respective owners.
This promotion is not affiliated with, endorsed by, or sponsored by the NFL, Levi’s Stadium, or any professional sports team.
11. DISPUTES
Except where prohibited, entrants agree that all disputes shall be governed by the laws of the State of New York, without regard to conflict of law principles, and that all proceedings shall be conducted in the state or federal courts located in New York County, New York.
